Credit Card on File:
A valid Credit Card is required to be kept on file. Nothing is charged on the card unless a patient NO SHOWS or has a LATE Cancellation ( <24hours of appointment time), then there will be a NON-REFUNDABLE $75 Fee that will be charged on the credit card on file. Patient consents to having the Credit Card on file be charged for any NO SHOW or LATE Cancellation Fees (cancellations <24hrs), or for any Deposits that are required for their specific treatment or procedure.
